A useful appointment note includes
Write the medication and injection date exactly as you remember it, the injection site if you track it, what you noticed afterwards, any questions you want answered and anything that made your routine harder to follow. Keep observations separate from conclusions.
Bring the questions you nearly forgot
Appointments move quickly. Note questions as they occur instead of trusting yourself to remember them later. Your clinician can tell you what is relevant, what needs attention and what should be recorded differently.
