How to make a free booking request form
A booking request form lets customers ask for an appointment at a time that suits them, and you confirm it. Website Maker has a Booking request template with the service, a preferred day and time, and contact details. It’s free, with no account, and each request arrives in your email, WhatsApp or texts.
A request, not a calendar
This is a request form, not a booking calendar: customers say when they’d like to come, and you reply to confirm or offer another time. It suits businesses that fit appointments around other work, such as dog groomers, tutors, cleaners, photographers and mobile hairdressers. If you need people to pick from your free slots and book instantly, you need a booking system with a calendar instead.
Make yours, step by step
- Open the Booking request template.
- Change the business name and the services to yours: tap Service and type one choice a line.
- Keep or change the questions: the day, the time, contact details, notes. Add your opening hours under the form.
- Choose where requests go: Email, WhatsApp or Text.
- Tap Share and put the link in your Instagram bio, Google profile or email signature.
How requests arrive
When someone presses Send, their own email app, WhatsApp or messages app opens with the request written out, for example:
Your dog’s name and breed: Biscuit, cocker spaniel
Day you’d like: 2026-11-20
Time you’d like: 10:30
Your name: Sam Rivera
Reply to it to confirm. To see every request in one list, paste them into More › Form replies, which makes a table and opens it in Gridwork.
Questions
Is it free?
Yes. Forms, the templates, every way to reply and Form replies are free. Website Maker Pro ($2.99 once) adds more pages, themes and fonts, no Yuzool line and a short link with visitor stats.
Will it stop double bookings?
No: it doesn’t know your calendar. You confirm each request yourself, so you choose who gets which time.
Can I take a deposit?
Not in the form. Send a payment link when you confirm the booking.
Can customers book from a QR code?
Yes. Share gives you a QR code to print for your shop window or van.
