How to Keep a GLP-1 Dose History Log for Appointments
Keep a clear personal history of GLP-1 injection dates, clinician instructions and questions so your next appointment starts with useful context.

A dose history log should help you remember what was prescribed and what happened, not tell you what to take next. The most useful record is dated, factual and easy to show to the qualified professional who manages your treatment.
Keep the medication name, dose and schedule exactly as provided by your clinician or pharmacist. If you are unsure whether a note is current, mark it as a question instead of guessing. Never use a tracker to change a dose or make a treatment decision.
Record the event and the context
For each entry, you may want to record the date, the instruction you were following and anything unusual about the day. If you noticed a symptom or local reaction, describe when it began, how long it lasted and what you want to ask about it. A short factual note is more useful than a conclusion about its cause.

Write the questions down before the appointment. Follow the answer from your clinician rather than a generic tracker or online discussion.
Keep the record private and deliberate
Health notes deserve care. Review what an export or share action contains before sending it, and only share it with the person who needs it. Keep official medication information and clinician instructions alongside your personal notes when relevant.
A calm review routine
- Add each event soon after it happens, while the details are fresh.
- Use dates and plain descriptions.
- Mark uncertainties as questions.
- Review the record before the appointment.
- Let a qualified professional make the medical interpretation.
